Disclosure Questions for Thrivent Member Network Board Members

Thrivent Member Networks of Thrivent Financial for Lutherans ("Thrivent Financial") are tax exempt organizations which are legally required to file information with the IRS annually. Thrivent Financial files a group Form 990 with the Internal Revenue Service annually for its Thrivent Member Networks which satisfies this filing requirement.

In order to meet IRS requirements, we need to ask questions of you on an annual basis. Keep in mind that a "yes" answer to a question does not mean that inappropriate conduct has occurred. We realize that "yes" answers to some of the questions would be highly unlikely, but we are required to ask the questions nonetheless.

Please review the definitions for the meaning of words and phrases in italicized bold type in the term definition list. All questions apply to transactions and relationships that occurred or were in effect during 2020. Include transactions that were initiated in a prior year, but continued in 2020.

* Relates to question 26 Part IV and Schedule L Part II Form 990: Did you or a family member of yours or a 35% controlled entity of yours loan to or borrow from your Thrivent Member Network any amount of cash in 2020? Charitable pledges and accrued but unpaid compensation owed by your Thrivent Member Network should not be included.

** Relates to question 28B Part IV and Schedule L Part IV Form 990: Did any family member of yours receive compensation in excess of $10,000 from your Thrivent Member Network in 2020, including payment for service as an employee or independent contractor?
** Relates to question 28C Part IV and Schedule L Part IV Form 990: Did you serve as an officer, director, trustee, or key employee of an entity that conducted business with your Thrivent Member Network involving sales, leases, licenses, services, loans, or other transactions resulting in payments of more than $10,000 in total from your Thrivent Member Network during 2020 or that was engaged in a joint venture with your Thrivent Member Network during the time that you were serving as an officer, director, trustee or key employee?
