Glossary of Terms
Assignment: An agreement by a physician or medical supplier to accept the Medicare-approved amount for covered services as payment in full. You must pay any coinsurance amount.
Automatic Claim Filing: Allows you to give permission to Medicare to send claims information directly to your Medicare supplement insurer.
Coinsurance: A cost-sharing requirement whereby you assume a portion or percent of the costs of covered services. It is the percent of the Medicare-approved amount that you must pay after you pay the deductible for Part A and/or Part B. In the Original Medicare Plan, the coinsurance payment is a percentage of the approved amount for the service (such as 20%).
Deductible: The amount of money you must pay before Medicare or other insurance providers begin paying for covered services. You must meet this deductible each calendar year or benefit period.
Excess charges: The difference between the Medicare-approved amount for a service or supply and the actual charge, if the actual charge is more than the approved amount, limited to 15% above the Medicare-approved amount.
Pre-existing condition: A Medicare supplement policy shall not exclude or limit benefits for losses incurred more than six months from the effective date of coverage because they involved a pre-existing condition. The policy or contract may not define a pre-existing condition more restrictively than a condition for which medical advice was given or treatment was recommended by or received from a physician within six months before the effective date of coverage.
