If you are enrolling for a Health Savings Account and are not performing a rollover or transfer of dollars from another account, please print and complete ONLY the Health Savings Account Application, the HSA Eligibility and Information Form and the Health Savings Account Supplement Form. All other forms listed are not required at account opening.
